6) Place of residence - ( PDF File - Address Proof - Elelctricity Bill / Municipal tax Receipt - size max 100 Kb )
7) Cancelled cheque with Name printed on it - (PDF file - size max 100 kb)
8) Latest Bank Statement - (PDF file with size max 100kb)
9) Shop act license or Business Certificate from Grampanchayat or Municipal Corporation - (PDF file with size max 100kb)
Documents required for the PTEC Registration -
same as 1) to 9) as above
10) Documents showing the start of the liability of PTEC like PWD reg certificate / Medical Degree Certificate(for doctors) / Shop act License ( for Shop Owner)
